Here are some questions to think about prior to your appointment:

• When and how did your problem begin?
• What were you able to do prior to your injury / condition that you struggle with now? (Home or Recreational Activities)?
• How often do you experience symptoms?
• How do your symptom changing over time (frequency or intensity)?
• What makes things better or worse?

On the day of your first appointment, we ask that you complete and sign the intake form below. This form is a PDF version that can be printed and filled out prior to your visit.
Please note, if you are unable to print out the PDF, we will provide a paper copy on the day of your appointment.

It is helpful if you bring a list of your medications and any surgeries or procedures you have had in the past.

Be prepared to move around a lot during your physical therapy session, with that in mind:

• please wear loose fitting clothing that allows you to freely move the body part being treated;
• please also wear supportive shoes for exercise in our gym

For example: If you have shoulder pain, bring a shirt that allows access to your shoulder from the front, back and side. If you have leg pain, bring shorts or loose fitting, non-restrictive pants. You are always free to bring a change of clothes. Please remember to wear comfortable shoes as well!

Your first session is called Initial Evaluation. What should you expect during this session?

• We will investigate and uncover as much as possible about your condition, your previous level of function, and how your current situation is affecting your life and functional levels.
• We will do a physical examination to take specific measurements to examine and pinpoint some impairments that may be causing your problem.
• We will put together a personalised treatment plan for you.

Please arrive on time. We cherish your time and that of other patients, and strive to keep the office running smoothy.

• A normal visit will last on average 45 to 60 minutes.
• For the Initial Evaluation (first visit) allow yourself between 1 and 1.5 hours.

Out of consideration for our therapists and other patients, please inform us at least 24 hours in advance if you need to change an appointment time. This ensures adequate time for other patients to schedule necessary appointments. It also allows us to maintain our flexible scheduling and accomodate any last minute scheduling requests you may need. Thank you for your understanding.
